OFFICE VISIT
First Visit
The eye is one of the most complex parts of the body.
The diagnosis and treatment of eye disease requires thorough examination.
Expect to spend up to two hours for your first visit or if you have been referred
for a specialty exam. We suggest you bring sunglasses because your pupils may be
dilated.
· Patient Forms
You may download your requested form to complete, print, and
bring with you to your appointment. This will allow your
registration process to go as smoothly as possible.
Insurance Authorization Many health insurance plans require authorization prior to seeing a specialist. Please check with your plan to avoid any inconvenience.
Payment Information We are happy to file your insurance claim with appropriate insurance card, workers comp claim number or Medicaid card and any required referrals upon your arrival. Co-payments are expected at time of service. We also accept Visa and
MasterCard for your convenience.
In Case of Emergency Our group provides medical coverage 24 hours a day.
In the event of an emergency after regular hours, please call
(937) 320-2020 and your message will be forwarded to our
physician-on-call.
